Illinois Association for Behavioral Health Survey

The Parity at 10 Campaign needs your assistance in disseminating a consumer survey to help understand consumer experiences when seeking mental health and substance use disorder (MH/SUD) treatment through insurance coverage. With 290 people dying every day from either drug overdose or suicide, your responses will help us improve access to treatment services.

Please disseminate this survey to any individuals who need or have sought SUD/MH treatment and live in Illinois. We also encourage parents, families and friends who have sought treatment for themselves or their loved ones for SUD/MH conditions to also complete this survey.


The survey can be accessed by clicking on and takes ~10 minutes to complete.

Your responses will help us better understand consumer and family experiences when seeking SUD/MH services through their insurance coverage and what consumers know about the federal Mental Health Parity and Addiction Equity Act – a federal law that bars discrimination in insurance coverage for these health conditions. The survey results will be used to develop educational materials and strategies to educate consumers and families about accessing in care for SUD/MH services and challenging denials of care.

The goal of the Parity at 10 Campaign is to expand access to SUD/MH services through strong enforcement of the Parity Act. The Illinois Parity at 10 Campaign is working with five national organizations—Legal Action Center, The Center on Addiction, The Kennedy Forum, Partnership for Drug-Free Kids, and Public Health Management Corporation—on this campaign. The national organizations have drafted this survey with input from IABH.
